Please login using the e-mail address on file and the temporary password (system assigned) that was e-mailed to you. If you do not have the temporary password or you need a new temporary password, please use the Forgot Your Password? link below. A new temporary password will be sent to you.

On your first log in, you will need to change the temporary password. The new password must contain at least 7 alphanumeric characters with at least one capital letter, one lower case letter and one number.

PLEASE NOTE: Internet Explorer 11 or above must be available on the device you are using for submission. The Practitioner Portal will not function correctly with previous versions. You may also use Chrome, Edge or Safari. Firefox is not a supported browser at this time.
